The RACKWiki Incident Reports project is a centralised, community‑driven database of adverse events, near‑misses, and safety incidents experienced during Risk‑Aware Consensual Kink (RACK) activities. By documenting what went wrong whether due to equipment failure, miscommunication, medical emergency, or human error we aim to provide real‑world data that helps practitioners make better‑informed risk decisions.

Purpose and guiding principles
- Learning, not shaming: Reports are published to educate, not to assign blame or humiliate individuals.
- Anonymity: Submitters may use pseudonyms. Any potentially identifying details are removed before publication unless the submitter explicitly consents to share them.
- Verification: While absolute verification is not always possible, moderators will attempt to confirm reports via follow‑up communication or cross‑referencing with other reliable sources.
- Consent: All reports must be submitted voluntarily. Do not submit a report on behalf of another person without their explicit permission.
How to submit an incident report
To submit a report, you may:
- Use the dedicated submission form (recommended), or
- Email the moderation team at incidents@rackwiki.example.com with a detailed description.
What to include:
- Activity involved (e.g., choking, electrostimulation, enema play, chemsex substance)
- Equipment used (make, model, material, modifications, if relevant)
- Description of incident (chronology, warning signs, immediate effects)
- Medical treatment sought (if any; include hospital diagnoses if available)
- Outcome (full recovery, ongoing issues, permanent injury, fatality)
- Lessons learned (what would you do differently?)
- Consent to publish (explicit permission is required; the moderator will confirm anonymity preferences)
