Records of incidents help the community understand the causes and can help prevent future occurrences. Reports, both fatal and not, may be self-submitted or gathered by RACKWiki volunteers. Case studies are in-depth analyses peer-reviewed by RACKWiki volunteers.
Incident reporting
Incidents can be reported using the Incident reporting form.
Case studies
Case studies are incidents selected for more thorough analysis with peer review. The goal is to provide details on what went wrong and how to prevent it from re-occurring.
